Technical Assistance:
Technical Assistance was discussed with the provider regarding:

-labels for infant bottles when more than one infant will be in care; requirements for menus and drill logs; and ensuring that all documents are back in the provider and household member records by the next inspection.

Comments:
An unannounced monitoring inspection was conducted on 3/28/23 from 10:25am to 11:45am with the provider. There were five children in care, ranging in age from six-months to five-years-old, for a total of 13 points. Children were observed having free play, napping, washing hands and having lunch, and playing with puppets with the provider. Four child records, the provider?s record, and one household member record were reviewed. The provider has current certification in CPR and First Aid. The provider does not administer medication and understands that they must obtain certification in Medication Administration Training (MAT) in order to give medication in the future. Lunch served was chicken nuggets, rice, broccoli, bananas, and milk. The emergency drill log, attendance, and menu were also requested. Violations:
Standard #: 8VAC20-800-830-C
Description: Based on review of documentation and interview, the provider did not ensure that documentation was maintained of emergency evacuation and shelter-in-place drills. Evidence: The drill log for the past year was requested during the inspection and the provider reported that they did not know where it was.

Plan of Correction: Moving forward I will make sure all drills are documented and I will keep it for one year.

Standard #: 8VAC20-800-920
Description: Based on interview, the provider did not ensure that when meals or snacks were provided by the family day home, that the menu for the current one-week period was dated or given to parents or posted or placed in an area accessible to parents. Evidence: The menu was requested during the inspection and the provider reported that there was no menu available for the current week.

Plan of Correction: Because I am on the Food Program I submit the menu at the end of each week to the Food Program but I let the parents know what their child ate each day. I will create a menu for each week moving forward and will make substitutions on it if anything changes.

Standard #: 8VAC20-800-60-A
Description: Based on review of five child records, the provider did not maintain an up-to-date record at the family day home for each enrolled child. Evidence: On 3/28/23 there was no record at the family day home for Child E.

Plan of Correction: 3/28/23 was the child?s first day and I sent the parent home with the documents they needed to complete for the file. In the future I will obtain the documents before children start.

Standard #: 8VAC20-800-90-A
Description: Based on review of five child records, the provider did not obtain documentation that all children had been adequately immunized before attending the family day home. Evidence: There was no documentation of immunizations on record for Child E, with a start date of 3/28/23.

Plan of Correction: I thought I had one month to obtain documentation for the child. Now I understand that I only have one month to obtain physicals.

Standard #: 8VAC20-800-550
Description: Based on observation and interview with the provider, the provider did not ensure that play pens were not used for a designated sleeping area. Evidence: On 3/28/23 Child E was observed using a play pen for their nap.

Plan of Correction: Child E?s parent asked me to use the play pen and provided it for their naps. 3/28/23 was the child?s first day. I will speak with the parent about using one of my cribs instead.
