Comments:
An unannounced monitoring inspection was conducted from 12:30 pm to 1:20 pm. At the time of entrance, 6 children totaling 14 points were in care with the provider. The record sample size consisted of 5 children's records and the provider's record. The children in care were observed engaging with the provider, handwashing, lunch, and nap. There have been no injuries since the last inspection and no medications are being administered at this time. Areas of noncompliance are noted in the violation narrative and were discussed with the provider during the exit meeting at the conclusion of the inspection.

Violations:
Standard #: 22.1-289.058
Description: Based on observation and interview it was determined the provider did not ensure that the home was equipped with at least one carbon monoxide detector.
Evidence:
1. There was no carbon monoxide detector in the home.
2. Provider confirmed that they did not have a carbon monoxide detector.

Plan of Correction: Provider will install a carbon monoxide detector in the home.
