Technical Assistance:

Inspector and provider discussed ensuring all staff have up to date trainings a total16 hours for the year, upcoming deadlines for background checks and changes in enrollment.

Comments:

An in-person mandated monitoring inspection was conducted on March 31, 2023 from 10:00am until 10:40am. There were 10 children in care (30pts) with 3 staff supervising. Staff and children were observed singing and dancing to music prompts that were playing. Children and staff were jumping, skipping and laying on the carpet. Once music time was completed, children and staff worked and played with large blocks to build a structure around the room. Children added different types of toys to their play cars and different animals. The older infant joined in the fun while being closely supervised by a caregiver. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 3 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Description:	Based on record review, children's records did not contain all required information. Evidence: Child 1 and 2 are missing a second emergency contact.
Plan of Correction:	We will contact parents for the second emergency contact information.
Standard #:	8VAC20-800-280
Description:	Based on observation, cleaning agents were stored in an area accessible to children. Evidence: In the bathroom utilized by children, cleaning products were stored in an unlocked lower cabinet.
Plan of Correction:	We will ensure the cabinet gets locked.
Standard #:	8VAC20-800-530-I
Description:	Based on observation, mattresses were not covered with a waterproof material that can be cleaned and sanitized. Evidence: In the sleeping room, four crib mattresses were found to be ripped and torn exposing the interior of the mattresses and could not be cleaned and sanitized.
Plan of Correction:	We will remove the mattresses we do not use.

Description:	Based on observation, the provider did not ensure the diapering surface was nonabsorbent. Evidence: The diaper changing pad in the common area was torn exposing the inner fabric and lining of the pad and could not be cleaned and sanitized.
Plan of Correction:	Changing pad was replaced at time of inspection.
