Technical Assistance:

Consultation was given on obtaining fictious name documentation from the SCC and the form sent to the provider. Guidance resources and training opportunities as well as model forms can be obtained from the updated Licensing website at www.childcare.virginia,gov. The provider was sent the weekly Readiness Connection that comes out every Tuesday so she can subscribe and get updates from Licensing weekly.

Comments:

An unannounced monitoring visit was conducted today. 9 children were present with 2 caregivers for 24 points which is within ratios for supervision. Observed infant care, free play, dancing, rest time for one child and children dancing to music. Reviewed a sample of records for children, physical space, provider training, nutrition, emergency preparation and attendance. Lunch to be served today was lentils, potatoes, rice and banana. One child is still on formula and bottles are prepared at the family day home. No medication was on-site today. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #:	8VAC20-800-100-A
Description:	Based on record review, 1 of 3 children's records reviewed did not have a physical exam record within 30 days of attendance. Evidence: Child #1 was missing a physical record-date of attendance 9/2022.
Plan of Correction:	The physical exam record was obtained from the parent.
Standard #:	8VAC20-800-60-B
Description:	Based on record review, 1 of 3 children's records was incomplete. Evidence: Child #2 was missing the health insurance policy number
Plan of Correction:	Provider will request the parent update the enrollment form.
Standard #:	8VAC20-800-240-A
Description:	Based on observation, areas of the home inside and outside were not maintained in a safe and operable manner. Evidence: 1. A smaii hole in the wall as visible in the large play room. 2. A broken glass vase was on the top of the shed roof by the garage/driveway.
Plan of Correction:	The hole was patched over the weekend and the broken glass vase was removed immediately.
