Technical Assistance:
Discussion of the FDH Renewal Application. This document was completed and emailed to the Fairfax office.

The Licensing Inspector (LI) will email the following to the FDH: Parents Information form, Medication Policy form for the FDH, Provisional of the EP&RP form, the Health Forms for the VA Health Department, and the Declaration of liability form. Most of these forms can also be found on our website: childcare.virginia.gov

Comments:
This renewal inspection was initiated by licensing staff and concluded on 04/05/2023, between the hours of 10 am to 12 pm. There were 2 children with 1 resident child present (8 points), ranging from infant to preschool-age, with 1 caregiver supervising. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; special care and emergencies; and nutrition. A total of 2 children records, 1 staff record, and 6 household records were reviewed.

Information gathered during the inspection determined that there were (5) non-compliances with the applicable standards, code, and/or law. These violations were documented on the Violation Notice issued to the home.

Violations:
Standard #: 8VAC20-800-800-A
Description: Based on review and interview, the Family Day Home did not have a written Emergency Preparedness and Response Plan (EP&RP) available.

Evidence -
1. On the date of inspection (04/05/2023), and EP&RP was not available for review.
2. The Provider stated (during an interview conducted on the date of inspection) that the current EP&RP could not be located.

Plan of Correction: An EP&RP will be completed and posted on main bulletin board.

Standard #: 8VAC20-800-100-A
Description: Based on review, 1 of 2 children files indicated that the Provider did not obtain documentation of a physical examination (by or under the direction of a physician) within 30 days after the first day of attendance.

Evidence -
1. On the date of inspection (04/05/2023), documentation of a physical examination for Child #1 was not available for review.
2. The start date for Child #1 was listed as 02/21/2023.

Plan of Correction: Documentation of a physical examination will be obtained.

Standard #: 8VAC20-800-60-A
Description: Based on review and interview, 1 of 2 children files indicated that the Provider did not maintain an up-to-date record at the Family Day Home (FDH) for each enrolled child.

Evidence -
1. On the date of inspection (04/05/2023), the child's record for Child #2 was not available for review.
2. The Provider stated (during an interview conducted on the date of inspection) that the forms were given to the parent of Child #2, but have not yet been returned. The Provider stated that the start date for Child #2 was 03/28/2023.

Plan of Correction: The Parent of Child #2 will be contacted and the documentation obtained.

Standard #: 8VAC20-800-70-A
Description: Based on review, 1 of 2 children files indicated that not all of the required documentation was completed and contained within each child's file.

Evidence -
1. On the date of inspection (04/05/2023) the following documents were not available for review within the file of Child #1: Information for Parents form, Provisional of the EP&RP form, Policy for the Administration of Medication form, and the Liability Declaration form.
2. The start date for Child #1 was listed as 02/21/2023.

Plan of Correction: The missing documentation will be obtained and placed on file.

Standard #: 8VAC20-800-90-A
Description: Based on review, 1 of 2 children files indicated that documentation that the child has been adequately immunized (according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations) prior to their start was not available.

Evidence -
1. On the date of inspection (04/05/2023), an immunization page was not available for review for Child #1.
2. The start date for Child #1 was listed as 02/21/2023.

Plan of Correction: An immunization page will be obtained for Child #1.
