An unannounced monitoring inspection was conducted on 4/10/2023. The inspector was on site at the family day home from approximately 10:40am to 12:25pm. There were 7 children in attendance and a total of 1 caregiver present. Interviews were held with the caregiver throughout the inspection, and the inspector interacted with children when appropriate. The yard and all areas of the home were inspected today. One caregiver/household member record and four children?s records were reviewed during this inspection. There were two violations found during today?s inspection. See the violation notice for more details.

Standard #: 8VAC20-800-770 Description: Based on observation, the provider did not ensure a working portable battery-operated weather band radio, and extra batteries were kept in a designated area and be available to caregivers at all times.
Evidence:
The provider did not have a working battery-operated radio.Plan of Correction: Will buy a new radio.
Standard #: 8VAC20-800-100-A Description: Based on record review, the provider did not ensure one of four children's records contained documentation of a physical examination by or under direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence:
The record of child #1 (enrollment date: 1/9/23) did not have documentation of a physical examination.Plan of Correction: Will obtain from parents today.
