Technical Assistance:
Provided consultation on standard:
1) 60B: All information is required on the child registration form. Encourage parents to use "N/A" rather than leaving a space blank if the information doesn't apply. Written authorization is required if the child is to be taken off of the premises of the family day home.
2) 120: The provider shall maintain a record for each caregiver that contains their name, address, verification of age, job title, date of employment or volunteering, name, address and phone number of a person to be notified in an emergency and documentation that two or more references as to character and reputation as well as competency were checked before employment. See standards for documentation to be kept regarding reference checks.
3) 200A: the provider shall orient the substitute provider and assistant by the end of their first week of assuming job responsibilities
4) 320: small electrical appliances such as coffee pots shall be unplugged unless being used. Child-resistant covers shall be installed on all unused electrical outlets and surge protectors that are accessible to children under five years
5) 480A: the family day home shall provide a sufficient quantity and variety of play materials and equipment that shall be readily accessible to children
6) 580C: Daily age-appropriate activities shall include: 1) opportunities for alternating periods of indoor active and quiet play depending on the ages of the children; 2) opportunities for vigorous outdoor play daily, depending on the weather; 3) opportunities for children to exercise initiative and develop independence; and 4) opportunities for structured and unstructured play time and provider-directed and child-initiated learning activities
7) 760A: the first aid kit shall include triangular bandages (indicating more than one)
8) 880: When the family day home provides meals and snacks, they shall follow the most recent, age-appropriate nutritional requirements of the USDA which includes milk at breakfast and lunch

Comments:
An announced initial inspection was conducted from 10:00 am - 12:30 pm with the provider and her assistant. There were four children in care, ranging from four years to five years of age, for a total of seven points. Children were observed working at their desks engaged in learning activities, eating lunch and napping. Four child records and five caregiver/household member records were reviewed. The childcare area was observed and is organized and well equipped with learning materials and some age-appropriate toys. Reviewed and discussed the written attendance, first aid kit, battery operated flashlight and weather band radio, emergency preparedness plan, weekly menu, required posted information, and child and caregiver records. Standard #: 8VAC20-800-770
Description: Based on observation, and interview, the home is not equipped with a working portable battery-operated weather band radio as required with extra batteries available. Evidence: the provider stated she did not have a weather-band radio.

Plan of Correction: A battery-operated weather band radio has been purchased

Standard #: 8VAC20-800-790
Description: Based on observation, telephone numbers for 1) 911 or local dial number for police, fire and emergency medical responders, 2) the responsible person for emergency backup care, or 3) the regional poison control were not posted in a visible area close to the telephone. Evidence: the provider did not have telephone numbers posted.

Plan of Correction: Emergency telephone numbers have been posted on the bulletin board.

Standard #: 8VAC20-800-920
Description: Based on observation and interview, the home did not have a menu for the one-week period that was: dated and given to parents or posted. Evidence: the provider stated that she did not have a menu for the week.

Plan of Correction: A dated menu that conforms to USDA requirements will be maintained and provided to parents weekly.

Standard #: 8VAC20-800-940-B
Description: Based on observation and interview, the home did not have an operable thermometer available to monitor the refrigerator and freezer temperatures. Evidence: the provider stated that she did not have a thermometer.

Plan of Correction: Thermometers have been purchased.

Standard #: 8VAC20-800-50-A
Description: Based on observation, and interview, the provider did not keep a written record of children in attendance. Evidence: there was no written record of attendance for the four children in care.

Plan of Correction: Provider stated she will maintain a written record of children in attendance.

Standard #: 8VAC20-800-280
Description: Based on observation, not all potentially poisonous substances were stored inaccessible to children in care. Evidence: there were two air fresheners in the bathroom, sitting in reach of children. One stated "keep out of reach of children" and "contents under pressure."

Plan of Correction: The air fresheners were removed from the bathroom.

Standard #: 8VAC20-800-330-A
Description: Based on observation, and interview, the home is not equipped with a landline telephone, excluding a cordless or cell phone, that is available and accessible during the family day homes' hours of operation. An operable landline telephone is one that does not require electricity to operate. Evidence: a landline telephone was not observed, and the provider stated that she did not have one.

Plan of Correction: A landline telephone has been purchased.

Standard #: 8VAC20-800-340-C
Description: Based on observation, the bathroom did not contain paper towels. Evidence: children were observed washing their hands and there were no paper towels in the bathroom.

Plan of Correction: Paper towels were placed in the bathroom.
