Technical Assistance:
A discussion was held in regards to the importance of maintaining the attendance record in an accurate and clear manner.

Comments:
A renewal inspection was conducted on 04/13/2023 between the hours of approximately 8:55 am and 10:05 am. There were 5 children present, ranging in ages from 12 months to 4 years (13 points), with the provider.

The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 2 staff/household member records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations:
Standard #: 8VAC20-800-770
Description: Based on review of emergency supplies, a working portable battery-operated weather band radio was not available.
Evidence:
1) On the date of inspection a small rechargeable portable radio was available to review but it was not charged, making it not-working.

Plan of Correction: The provider shall ensure to have a working battery-operated weather-band radio.

Standard #: 8VAC20-800-780-B
Description: Based on record review, the provider did not review with the parent, annually, the emergency contact information to ensure the information is correct and obtain the parent's signed acknowledgment of the review.
Evidence:
1) The most recent review of the emergency information in the record for Child #1 was dated 03/13/2022.

Plan of Correction: The provider shall request the updated information to the parents of Child #1.

Standard #: 8VAC20-800-100-A
Description: Based on record review, the provider did not ensure to obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence:
1) The record for Child #2 (start date 06/20/2022) did not contain the results of a physical examination.

Plan of Correction: The provider shall communicate with the parents to request a physical examination for Child #2.

Description: Based on record review, the provider did not obtain the parent's written acknowledgement of the receipt of required information.
Evidence:
1) The record for Child #1 (start date 03/14/2022) did not contain a signed acknowledgement of having received information on whether or not there is liability insurance on the family day home.

Plan of Correction: The provider shall provide parents of Child #1 the required information and obtain their written acknowledgement.

Standard #: 8VAC20-800-180-A
Description: Based on household member record review, the provider did not obtain a current Report of Tuberculosis (TB) Screening every two years from the date of the first screening.
Evidence:
1) The most recent tuberculosis screening in the record for household member #1 was dated 08/15/2019.

Plan of Correction: The provider shall ensure that HM#1 completes a Tb screening.

Standard #: 8VAC20-800-250-E
Description: Based on observation, the provider did not ensure that plastic bags large enough for a child's head to fit inside were inaccessible to children under five years of age.
Evidence:
1) A plastic bag was observed inside the shower area of the bathroom used by children.

Plan of Correction: The provider removed it during the inspection.

Standard #: 8VAC20-800-280
Description: Based on observation, potentially poisonous substances, were not stored away in areas inaccessible to children.
Evidence:
1) In the bathroom used by children, the following substances were observed in the under the sink cabinet and inside the drawers of the cabinet: 2 cans of Lysol spray, perfumes, lotions, makeup and a fragrance mist. The following was observed inside the shower area: Lysol wipes. A bottle of "Fabuloso" cleaner was observed on the floor, behind the toilet.
2) In the care area, a bottle of hand sanitizer and Lysol wipes were observed behind the t.v. in an area that can be reached by children.

Plan of Correction: During the inspection, the provider removed the substances.

Standard #: 8VAC20-820-120-E-2
Description: Based on review of required posted documents, the provider did not ensure that all required documents related to the terms of the license were posted on the premises of the program.
Evidence:
1) On the date of inspection, the findings of the most recent inspection of the facility (12/06/2022) were not posted. The inspection findings posted were dated 11/12/2021.

Plan of Correction: The provider shall ensure to post the results of this inspection once it is received.
