Comments:
A monitoring inspection was conducted on April 11, 2023. There were six children present with three staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of four child records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-800-60-B
Description: Based on review, provider did not ensure 4 of 4 children's records has documentation of required information.

Evidence: The following was found during review of children's records:
1. The record for child #1 did not have first date of attendance, parent's employment address and phone number, emergency contact address, medical insurance policy number and parent's signed acknowledgment of the written information provided to parents (required information about program, liability insurance coverage and provision of provider's emergency preparedness/response plan).
2. The record for child #2 did not have first date of attendance, physician's address and parent's signed acknowledgment of the written information provided to parents (required information about program, liability insurance coverage and provision of provider's emergency preparedness/response plan).
3. The record for child #3 did not have first date of attendance, employment address and phone number for one parent and parent's signed acknowledgment of the written information provided to parents (required information about program, liability insurance coverage and provision of provider's emergency preparedness/response plan).
4. The record for child #4 did not have first date of attendance, employment phone number for one parent, address for one emergency contact, insurance policy number and parent's signed acknowledgment of the written information provided to parents (required information about program, liability insurance coverage and provision of provider's emergency preparedness/response plan).

Plan of Correction: Provider will take the forms out on a clipboard today and have parents complete the information.
