Comments:
A monitoring inspection was initiated and concluded on 4-13-2023.? There were 4 children present plus 2 resident children with the provider and 1 staff supervising. The point total was 10. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 1 staff record were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-800-730-D
Description: Based on observation and staff interview the licensee did not ensure medication, including refrigerated medication and medications for caregivers and household members, shall be kept in a locked place using a safe locking method that prevents access by children.
Evidence: A bottle of Benadryl and a bottle of Nyquil were observed in the door of the refrigerator that children had access to. The provider confirmed that the medications were not locked up.

Plan of Correction: Provider has relocated medicine from the refrigerator.

Description: Based on document review and staff interview the licensee did not ensure emergency evacuation procedures were practiced monthly with all caregivers and children in care during all shifts that children are in care.
Evidence: The provider confirmed there was no documentation that an evacuation drill was conducted in March 2023.

Plan of Correction: Provider will ensure that each evacuation procedure conducted is documented.

Standard #: 8VAC20-800-830-B
Description: Based on record review and staff interview the licensee did not ensure Shelter-in-place procedures shall be practiced a minimum of twice per year.
Evidence: The provider confirmed there was no documentation that at least two shelter in place drills were conducted in 2022.

Plan of Correction: Provider will ensure that all Shelter in place and emergency drills are documented on the same document

Standard #: 8VAC20-800-920
Description: Based on observation and staff interview the licensee did not ensure when meals and snacks are provided by the family day home, the menu for the current one-week period shall be dated and list any substituted food.
Evidence: The provider confirmed that the current week menu was not dated and the food listed for lunch was not the food that was served and the substituted food was not listed on the menu.

Plan of Correction: Provider will ensure that each week food menu is dated, and the food served does not deviate from the menu.

Standard #: 8VAC20-800-180-A
Description: Based on record review and staff interview the licensee did not obtain for each caregiver and adult household member a current Report of Tuberculosis Screening form, every two years from the date of the first screening.
Evidence: The provider confirmed that Staff #1?s most recent TB screening was dated 7-24-2020, and more than two years old.

Plan of Correction: Provider will have TB test scheduled for her and other household members.

Standard #: 8VAC20-800-320-G
Description: Based on document review and staff interview the licensee did not ensure fireplaces and associated chimneys shall be inspected annually by a knowledgeable inspector to verify that the devices are properly installed, maintained, and cleaned as needed.
Evidence: The provider confirmed they have not had the chimney inspected and it is not sealed off.

Plan of Correction: The Fireplace and Chimney will be inspected by a professional.

Standard #: 8VAC20-800-680-B
Description: Based on observation and staff interview the licensee did not ensure that children?s hands are washed with liquid soap and warm running water before eating
Evidence: Staff was observed handing lunch to the children. Staff confirmed they did not have them wash their hands before eating.

Plan of Correction: The children washed their hands. While waiting for lunch they started to play again and did not wash their hands again. Provider will make sure children wash their hands when lunch is plated.

Standard #: 8VAC20-820-120-E-2
Description: Based on observation and staff interview, the licensee did not ensure the results of the most recent inspection required by the Superintendent were posted in a conspicuous place on the licensed premises.
Evidence: The provider confirmed that the most recent inspection results dated April 4, 2022 were not posted.

Plan of Correction: The last report was posted in a binder. The most recent inspection results were immediately posted.
