Technical Assistance:
The LI and provider discussed posting violation notices, menu dates, outlet covers (even if out of reach of children), no pillows for children under two years, topical products, and reviewing registration records for completeness (all sections contain required information). The LI sent a link to the provider to document that the parents have been informed of the provisions of the Emergency Plan.

Comments:
A monitoring inspection was conducted on 4/5/2023 from 11:40 am to 1:25 pm. Four children, ages 2 months to 2 years (total of 12 points) were in care with one caregiver. The Licensing Inspector reviewed 2 children?s records and other program records. The children were observed watching TV and playing.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-800-60-B
Description: Based on record review, the provider failed to ensure that child records contained some required information.

EVIDENCE:
1. The record for Child 1 did not contain the doctor?s phone number, insurance information, one of the emergency contacts didn?t have an address, and there was no authorization for the provider to obtain emergency care.

2. The record for Child 2 had no information regarding prior school, allergy information, health problems, an emergency contact did not have a complete address, and there was no authorization for the provider to obtain emergency care.

Standard #: 8VAC20-800-80-A
Description: Based on record review, the provider failed to obtain proof of age and identity within seven business of the child?s first day of attendance.

EVIDENCE:
1. The record for Child 1 shows attendance began on 3/21/2023 and did not contain proof of age or identity.

2. The record for Child 2 shows attendance began on 3/22/2023 and did not contain proof of age or identity.

Standard #: 8VAC20-800-90-A
Description: Based on record review, the provider failed to obtain immunization records showing children have been adequately immunized prior to attending the family day home.

EVIDENCE:
1. The record for Child 1 did not have immunization records.

2. The record for Child 2 did not have immunization records prior to the child attending. Child 2 attended 3/22/2023 and the immunization records were obtained 3/23/2023.
