Evidence: Child #5's record does not contain the families medical insurance policy number.Plan of Correction: I will have the family fill it out.
Standard #: 8VAC20-800-180-A Description: Based on interview, the provider did not obtain a subsequent tuberculosis (TB) screening every two years for each employee.
Evidence: The most recent documented TB screening for Caregiver #3 is dated 4/16/2021. Caregiver #3 confirmed that they had not yet received a repeat TB screening.Plan of Correction: I will speak with [them] about making an appointment.
