Technical Assistance:
Technical Assistance was provided and licensing standards were reviewed regarding household members files, TB screenings, and standards regarding children's records.

Comments:
A renewal inspection was conducted on April 17, 2023. The inspectors arrived at 1:40pm and departed at approximately 3pm. Present in the home were 8 children, totaling 22 points, and two providers. The youngest child in care was 1 years old. Children were observed bottle feeding, naptime, diapering, and being picked up by their parent. First Aid kits and emergency preparedness documents were observed. A sample size of five children's records were reviewed, two staff records and household members. It was reported that no medication has been administered to children in care and no children's injuries were reported. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented. The violations are listed on the violation notice issued to the center and were reviewed with the two providers at the exit interview.

Violations:
Standard #: 8VAC20-770-40-C-1-c
Description: Based on record review and interview, it was determined that the provider did not ensure that any other adult living in the home shall have a criminal history record check upon licensure.
Evidence:
1. Household member # 1 did not have the required criminal history record check for Virginia in their record.
2. Staff #1 and Staff #2 confirmed that Household member #1 did not have the required criminal history record check.

Plan of Correction: The provider responded with the following: Staff will ensure that a background criminal history check will be completed for Household member #1 and sent a copy of the results to licensing.

Standard #: 8VAC20-800-100-A
Description: Based on record review and interview, it was determined that the family day home did not ensure that documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance is in the record.
Evidence:
1. Child #1 (date of enrollment 6/14/22) did not have documentation of a physical in their record.
2. Staff #1 and Staff #2 confirmed that Child #1 did not have documentation of a physical examination in their record.

Plan of Correction: The provider responded with the following: Staff will obtain documentation of Child #1?s physical examination and place it in their record. Staff will go through all records to ensure that all physicals are in the children's records.

Standard #: 8VAC20-800-530-A
Description: Based on observation and interview, it was determined that the provider did not ensure that cots when being used for sleeping or napping by children other than infants shall have linens consisting of a top cover and a bottom cover or a one-piece covering that is open on three edges.
Evidence:
1. During naptime, all 5 cots did not have the linens consisting of a top cover and a bottom cover.
2. Staff #1 and Staff #2 confirmed that the children were not using linens consisting of a top cover and a bottom cover.

Plan of Correction: The provider responded with the following: The provider will ensure that all cots for the children will have the appropriate linens.

Standard #: 8VAC20-800-530-G
Description: Based on observation and interview, it was determined that the provider did not ensure that children under two years of age shall not use filled comforters.
Evidence:
1. Child #2, age 13 months, had a filled comforter blanket in their crib.
2. Staff #1 and Staff #2 confirmed that the child uses a filled comforter blanket.

Plan of Correction: The provider responded with the following: The provider will no longer use the comforter filled blanket with Child #2 and removed the blanket from the crib during the time of the inspection.
