The inspection was initiated on April 21, 2023. There were 10 children present with 3 staff supervising.
The inspector reviewed compliance in the areas of administration, personnel, physical health of caregivers, caregiver training, physical equipment/ environment, care of children, preventing spread of disease, emergencies and nutrition. A total of 3 child records, 3 provider record,
and 2 family member record were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the
violation notice issued to the program.

Standard #: 8VAC20-800-100-A Description: Based on record review and staff interview, the licensee did not ensure to otain a physical examination by or under the dorection of physicain prior to child's attendance or with in 30 days after the first day of attendance.
Evidence:
Child #1 (start date 06/13/2022) record was missing a physical . Child #1 has been attending for more than 30 days.Plan of Correction: Contacted parent on 04/19/2023
Parents scheduled physical w/doctor for last week of April 2023.
