Comments:

An unannounced monitoring inspection was conducted today from 1;20-2:00. There were 5 children for a total of 14points directly supervised by the Provider. The home, staff records, 2 children?s records, 3 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed napping and 2 were awake and with the Provider.. Diapering procedures were also observed. There was an assortment of age appropriate materials for children. The home was clean and organized. Areas of non-compliance are identified in the Violation Notice. Standard #:	22.1-289.035-A
Description:	Based on record review and interview, it was determined the Provider did not obtain, before 5 years had lapsed, a background check. Evidence: 1. Background check for the Provider is dated April 6, 2018. It has been 5 years since Provider went to obtain a finger print clearance.
Plan of Correction:	Will call and schedule. I did not realize it was close to expiring. I did obtain a Central registry Search but just missed the date.
Standard #:	22.1-289.036-A
Description:	Based on record review, it was determined the Provider did not have household member #1 undergo a finger print background check before 5 years had lapsed from the last one. Evidence: 1. Household #1's finger print clearance was dated April 19, 2018 and Provider stated the household member has not obtained a current one.
Plan of Correction:	Will call and schedule. I did not realize it was close to expiring. I did obtain a Central registry Search but just missed the date on this background check by a day.
