Technical Assistance:

The provider's emergency preparedness plan was reviewed. An older model form was used and adapted to add lock-down procedures. The inspector will email an updated model emergency preparedness plan form for the provider to use or adapt as desired.

Comments:

A renewal inspection was conducted on 04/18/2023. There were 7 children present, totaling 19 points, with 2 caregivers supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, special care and emergencies and nutrition. The children had eaten lunch prior to the inspection. Diapers were being changed while children playing before going down for nap. A total of 5 child records and 2 caregiver records were reviewed. The provider reported no children's injuries since the previous inspection. No medication is administered to the children. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations were reviewed with the provider during the exit interview.

Standard #:	22.1-289.036-B-2
Description:	Based upon review of caregivers records and provider interview, the provider has not ensured that the results of fingerprinting are obtained for all caregivers. Evidence: 1. The provider was unable to find the results of fingerprinting for caregiver 2 during the inspection. 2. The provider acknowledged that the results of fingerprinting for caregiver 2 were not in the records.
Plan of Correction:	The provider responded with the following: The assistant will immediately schedule an appointment to obtain new fingerprinting if the Office of Background Investigations no longer has the results of her previous fingerprinting in 2019.
Standard #:	8VAC20-800-280
Description:	Based upon observation, the provider has not ensured that potentially poisonous substances such as cleaning agents, sanitizers and pesticides are stored away from food in areas inaccessible to children. Evidence: 1. There was a container of Raid pesticide on the counter in the kitchen. 2. After diapers were changed, there was a container of disinfectant left on a dresser inside a bedroom door beside where the diapers had been changed in the hallway. 3. There was a plug-in liquid air freshener in an electrical outlet that was approximately 1 foot from the floor in the child care room.
Plan of Correction:	The provider responded with the following: All chemicals or hazardous substances will be kept in a curtained closet in the kitchen, away from food and inaccessible to the children in care.
Standard #:	8VAC20-800-560-A
Description:	Based upon observation and provider interview, the provider has not ensured the direct care and supervision of each child including the awareness of and responsibility for each child in care in order to intervene, if needed. Evidence: 1. Child 1, two years old, was observed running around the child care room and climbing on furniture during the entire inspection. She climbed up the outside of a high chair and onto the serving tray three times during the inspection. She was also observed running around and over other children while pushing them out of her way. 2. The provider acknowledged child 1's behavior and the challenge presented in ensuring her safety and the safety of the other children.
Plan of Correction:	The provider responded with the following: If a child's behavior presents a supervision challenge and a potential hazard to him/herself or the other children in care, a specific plan of action will be developed with the parent and/or the child will be terminated from care.
