Technical Assistance:

Discussed with the provider: emergency preparedness plan must be reviewed annually and review must be documented, physical examination for Child #1 must be obtained before the child?s 30th day of attendance, provider reminded to dump standing water in outdoor play equipment after rain

Comments:

An unannounced renewal inspection was conducted on 04/25/2023 between the hours of approximately11:00 a.m. and 12:15 p.m. There were 10 children present with the Provider and 1 assistant for a total of 32 points within the supervision guidelines. The children were observed eating, playing with blocks and cars and engaging in free play. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children, caregiver and household member records, emergency drill log, attendance and emergency procedures were observed during this inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Description:	Based on record review, the provider did not document in writing each review and update to the emergency plan. Evidence: The most recent documented review of the emergency plan was dated 10/18/2018.

Description:	Based on observation, high chair safety straps were not used and securely fastened, when occupied by a child. Evidence: Safety straps on 3 out of 3 high chairs observed, occupied by Child #1, #4 and #5, were not used and securely fastened.

Description:	Based on record review, an assistant?s record did not contain all information. Evidence: Caregiver #2?s (start date: 04/03/2023) record did not contain documentation that two or more references were checked before employment.

Description:	Based on observation, potentially poisonous substances were in areas accessible to children. Evidence: One hand cream (warning label states to keep out of reach of children) and one eczema cream (warning label states to keep out of reach of children) were in an unlocked drawer, next to the basement kitchen sink, accessible to children.
