Comments:
An unannounced renewal inspection was conducted today between the hours of 11:15am-1:30pm. There were 7 children directly supervised by the provider and 2 additional caregivers. The home, 3 caregiver records, 3 children?s records, 2 household member?s records, evacuation drills, and emergency supplies were inspected. Children were observed finishing lunch, playing freely, and participating in storytime. Handwashing procedures were also observed. Areas of non-compliance are identified in the Violation Notice.

Standard #: 8VAC20-800-800-B
Description: Based on record review, the provider did not document in writing each review and update to the emergency plan annually.

Evidence:
The most recent review of the emergency plan documented was dated 3/3/2021.

Plan of Correction: The plan was reviewed but I forgot to document it.

Standard #: 8VAC20-800-830-C
Description: Based on record review, documentation of evacuation drills were not maintained.

Evidence:
1.The evacuation drill log for the Family Day Home did not contain documentation of evacuation drills practiced for the months of February 2023 and March 2023.
1. Provider stated the drills were completed, but not documented.

Plan of Correction: The drills were completed but not documented. We will do another drill tomorrow.

Standard #: 8VAC20-800-120-B
Description: Based on record review, 1 of 3 caregiver records reviewed did not contain all required information.

Evidence:
1. The record for Caregiver #1 did not contain the following:
-Verification of age;
-Date of employment or volunteering;
-Name address and telephone number of a person to be notified in an
emergency;

Plan of Correction: I will get the information today.

Standard #: 8VAC20-800-60-B
Description: Based on record review, 1 of 3 children's records reviewed did not contain all required information.

Evidence:
The record for Child #1 did not contain the following:
- policy number of the child's medical insurance
-address for one of the two designated persons to contact in case of an emergency if the parent cannot be reached.

Plan of Correction: I will ask for the information today.

Standard #: 8VAC20-800-170-B
Description: Based on record review, the tuberculosis screening for 2 of 3 caregiver records reviewed were not obtained within 30 days of the first date of employment.

Evidence:
1. Provider stated Caregiver #1 began employment in February 2023. The documentation of tuberculosis screen/testing in the record for Caregiver #1 was dated 12/21/2022.
2. Caregiver #2 began employment on 1/23/2023. The documentation of tuberculosis screen/testing in the record for Caregiver #2 was dated 6/16/2022.

Plan of Correction: I will make an appointment by the end of the month.

Standard #: 8VAC20-800-180-A
Description: Based on record review, the provider did not obtain a current report of tuberculosis screening every two years from the date of the first screening for 1 caregiver.

Evidence:
1. The most current tuberculosis screening in the record for Caregiver #3 was dated 09/28/2020.

department.
