Technical Assistance:
Discussed maintaining the outside by making sure green carpeting is securely on ground and does not create a tripping hazard. Also noted ongoing checks of the fence is necessary to avoid having deteriorating wood and or exposed screws as a problem.

Comments:
An unannounced monitoring inspection was conducted today from 9:05am-10:20am. There were 6 children for a total of 16 points directly supervised by 2 staff. The home, 3 staff records, 3 children?s records, 1 household member?s record, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely inside and outside. There was an assortment of age appropriate materials for younger children but may want to have more for the older children. The home was clean and the Provider recently painted the children's area. The Inspector suggested that the Provider use a system that ensures all paperwork and documents are kept in one location so records are accessible and maintained for easy review. Areas of non-compliance are identified in the Violation Notice.

Violations:
Standard #: 22.1-289.035-A
Description: Based on record review, it was determined the Provider did not obtain a repeat Central registry search for a caregiver.
Evidence:
1. Caregiver #3 has a CPS check dated March, 2018.
2. Clearances need to be resubmitted every 5 years.
3. Provider stated she did not realize it had been 5 years since last check.

Plan of Correction: I will have caregiver enter info and have it complete.

Standard #: 8VAC20-800-770
Description: Based on observation, it was determined the Provider does not have a working battery operated weather band radio.

Evidence:
1. Provider has a radio but it only worked when plugged into the electrical outlet.
2. Provider placed batteries in the radio but it didn't work. She tried another set of batteries and those didn't work either.

Plan of Correction: I will buy more batteries as I didn't realize they were not working.

Standard #: 8VAC20-800-340-C
Description: Based on observation and interview, it was determined the bathroom did not contain paper towels.
Evidence:
1. Bathroom was checked and there were no paper towels. The Provider opened the closet next to bathroom and pulled out a package of facial tissues. Provider stated she though she had paper towels in the closet but would use these tissues.

Plan of Correction: I will grab paper towels from upstairs.

Standard #: 8VAC20-800-530-B
Description: Based on observation, the Provider is using cribs that do not have tight fitting bottom covers.
Evidence:
1. The Provider has three cribs and two of the cribs have linen covers that are too big and do not tightly fit on the mattresses.

Plan of Correction: I have other fitted sheets and will replace these.
