Discussion that Household Member #2 will be turning 18 years of age within the next year; FPC, SDS, and TB documentation will be needed. In addition, Household Member #3 will turn 14 years of age; a check of the VA central registry (CPS) will be needed.
Discussion of possible age range modification. Please submit this request by email with specific new age range and date you wish it to go into effect.
Licensing Inspector will send a copy of the following documents to Provider: SDS, Drill Log, and a copy of the FDH Renewal Application.

This monitoring inspection was initiated by licensing staff and concluded on 04/25/2023, between the hours of 1:30 pm to 3 pm. There was 1 preschool-age child present (2 points) with 1 caregiver supervising. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; special care and emergencies; and nutrition. A total of 2 children records, 1 staff record, and 3 household records were reviewed.
Information gathered during the inspection determined that there was (1) non-compliance with an applicable standard. This violation was documented on the Violation Notice issued to the home.

Standard #: 8VAC20-800-100-A Description: Based on review, 1 of 2 children files indicated that the Provider did not obtain documentation of a physical examination (by or under the direction of a physician) within 30 days after the first day of attendance.
Evidence -
1. On the date of inspection (04/25/2023), documentation of a physical was not available for Child #2.
