Comments:
A monitoring inspection was initiated on April 25, 2023 and concluded on April 26, 2023. There were three children present with one caregiver supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of three child records and one caregiver record were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-800-770
Description: Based on observation, provider did not ensure there is a working battery-operated flashlight, a working battery-operated weather band radio and extra batteries.

Evidence: The flashlight and weather band radio were not operable. Provider stated she needed to get batteries.

Plan of Correction: Will get batteries for the flashlight and radio.

Standard #: 8VAC20-800-830-C
Description: Based on review, provider did not ensure emergency drill documentation has required information.

Evidence: The emergency evacuation drill conducted in August 2022 did not document the date of the drill.

Plan of Correction: Dates for all drills will be documented as they are done.

Standard #: 8VAC20-800-920
Description: Based on observation, provider did not ensure menus posted are dated for the current week.

Evidence: The menu posted was dated 2/19/23 - 2/25/23 which is not the current week.

Plan of Correction: Will print an updated menu today.

Standard #: 8VAC20-800-50-A
Description: Based on review and observation, provider did not ensure there is a written record of children in attendance each day.

Evidence: Children's attendance was not documented for this day or the day prior. Three children were observed present on April 25, 2023.

Plan of Correction: Attendance will be documented.

Standard #: 8VAC20-800-60-B
Description: Based on review, provider did not ensure 1 of 3 children's record has documentation of required information.

Evidence: The record for child #1 did not have documentation of first date of attendance, immunization record and proof of identity.

Plan of Correction: First date of attendance was added to the record during inspection. The immunization record and birth certificate will be found and put in record.

Standard #: 8VAC20-800-210-A
Description: Based on review, provider did not ensure in addition to first aid training and CPR training, caregivers complete a minimum of 16 hours of training annually.

Evidence: Provider had documentation of completing 10 hours of training in 2022.

Plan of Correction: Training has begun for 2023. Will make sure at least 16 hours is completed and training certificates filed in provider's record.

Standard #: 8VAC20-800-240-A
Description: Based on observation, provider did not ensure areas and furnishings of the family day home, inside and outside, are maintained in a clean, safe and operable condition.

Evidence: The following was observed in the childcare room during inspection:
1. An electrical cord from a mounted ceiling projector was stretched down and plugged into a outlet. The cord was within reach of children.
2. An electrical cord was hanging from a heater sitting on top of an upper cabinet. The cord was hanging down within reach of children.
3. Approximately 15 cardboard boxes were stacked on top of a two drawer file cabinet reaching up to the ceiling. The stack was leaning and within reach of children.
4. Provider stated the children are not allowed in this area however there was not a barrier to prevent access by children.

Plan of Correction: A new bookshelf will be put in so all cords will be covered.
