Technical Assistance:

The provider must send documentation to the Licensing Inspector that the background checks have been requested no later than 10 days following this notification dated 05/01/2023. Once the background check is received by the center, the results must be forwarded to the licensing inspector.

Comments:

An unannounced monitoring inspection was conducted on May 1,2023. There were 11 children present (29 points) with 2 caregivers. The home, children's records, caregiver records, household member records, evacuation logs, attendance logs, menus, first aid kits and cribs were reviewed. Also, outside play area was reviewed. There were no medications on site. The children were observed playing with floor toys and playing outside. Areas of non-compliance were cited on the violation notice. Description:	Based on a review of caregiver records, the provider failed to submit to fingerprinting and provider descriptive information. Evidence: Caregiver #1 fingerprints were obtained 02/02/2018 and expired 02/02/2023.
Plan of Correction:	Within the next ten days, I will have my fingerprints completed.
Standard #:	8VAC20-770-40-D-4-a
Description:	Based on a review of Household Member records, the provider failed to obtain current Criminal History Background Records checks. Evidence: Household Member #1, #2 and #3 Criminal History Background Checks have expired. Household Member #1 and #2's Criminal History Records check expired 02/07/2023 and Household Member #3's Criminal History Records check expired 02/12/2023.
Plan of Correction:	I will have all household members complete the fingerprint process.
Standard #:	8VAC20-800-780-B
Description:	Based on a review of children records, the provider failed to review with the parent the emergency contact information required in 8VAC20-800-60-B to ensure the information is correct and obtain the parent's signed acknowledgement of the review. Evidence: Child #2's record does not contain documentation of an annual review of emergency contact information. Child #2's last review was 07/28/2021.
Plan of Correction:	I will complete the annual review and will remember to complete reviews annually.
Standard #:	8VAC20-800-80-A
Description:	Based on a review of children's records, the provider failed to obtain, within seven day of the child's first day of attendance at the family day home, verification of the identity and age of the child. Evidence: Child #3 and Child #5's records did not contain documentation of verification of identity within seven business days. Child #3's first day of attendance was 06/22/22 and Child #5's first day of attendance was 04/07/2023.
Plan of Correction:	I will get the information from the parents and put in the child's record
Standard #:	8VAC20-800-180-A
Description:	Based on a review of caregivers and household member records, the provider failed to obtain a current Report of Tuberculosis Screening form, in accordance with the requirements in 8VAC20-180-170, every two years for the date of first screening. Evidence: Caregiver #1's record does not contain a current tuberculosis screening, the last screening was 04/15/2021. Household Member #1, #2 and #3's records do not contain documentation of a current tuberculosis screening. Their last screenings were HHM #1 - 4-04/15/2021, HHM#2-0427/2021, HHM#3-04/26/2021.
Plan of Correction:	I will have caregiver's and household members complete tuberculosis screenings. And will ensure screening are completed every two years.
