Comments:

A renewal inspection was conducted on 05/02/2023 between the hours of approximately 9:10 am and 10:00am. There were 4 children present with the Provider supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child and 1 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #:	8VAC20-770-40-D-1-a
Description:	Based on record review and interview, it was determined that the Provider did not ensure that Central Registry search results are obtained every five years. Evidence: 1) The most recent Central Registry search request results available in the record for the Provider was dated 01/30/2018.
Plan of Correction:	The provider shall submit a central Registry search request within 10 days.
Standard #:	8VAC20-770-60-B
Description:	Based on record review, one child's record did not contain all required information. Evidence: 1) The record for Child #1 did not contain the name, address and telephone number of a second designated person to contact in case of an emergency if the parent cannot be reached
Plan of Correction:	The provider shall request the parents of Child #1 the missing information.
Standard #:	8VAC20-800-100-A
Description:	Based on record review, one child record did not contain a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Evidence: 1) The record for Child #1 (start date 01/03/2023) did not contain the results of a physical examination.
Plan of Correction:	The provider shall request the parents of Child #1 to provide the results of a physical examination ASAP.
Standard #:	8VAC20-800-210-A
Description:	Based on record review, caregivers did not obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities. Evidence: 1) On the date of inspection, the Provider had completed 1 hour out of the required sixteen clock hours of annual training during the May 2022 - May 2023 license year.
Plan of Correction:	The provider shall ensure to complete the required annual training hours.
Standard #:	8VAC20-800-320-G
Description:	Based on documentation review, the provider did not ensure that wood burning fireplaces and associated chimneys were inspected annually by a knowledgeable inspector to verify that the devices are properly installed, maintained, and cleaned as needed. Evidence: 1) The most recent fireplace inspection was dated 12/14/2019.
Plan of Correction:	The provider shall request a fireplace inspection ASAP.
