A renewal inspection was completed on May 2, 2023 from 9:50am to approximately 11am. There were no children present at the time of the inspection however there are three children currently enrolled. The Inspector reviewed the provider?s record and the three children?s records. Emergency preparedness documents were reviewed as well as the providers training log. Information gathered during the inspection determined non-compliance with applicable standards or law and a violation was documented on the violation notice issued to the program. The violation was discussed with the provider at the exit interview.

Standard #: 8VAC20-800-90-A Description: Based on record review and interview, it was determined that the provider did not ensure that before a child may attend the family day home, the provider shall obtain documentation that the child has been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations.
Evidence:
1. Child #1?s (date of enrollment 1/09/2023) record did not have documentation completed to demonstrate that they have been adequately immunized as required.
2. Child #2?s (date of enrollment 1/09/2023) record did not have documentation completed to demonstrate that they have been adequately immunized as required.
3. Staff #1 confirmed that there was information missing from immunization documents in the records of Child #1 and Child #2.Plan of Correction: The provider responded with the following: The provider called the father during the inspection and left a message requesting completed immunization documents for both children?s records.
