Violations:
Standard #: 8VAC20-800-90-C
Description: Based on record review, the family day home shall obtain documentation of additional immunizations for each child under the age of two years.
Evidence: Child #3 (17 months) most recent documented immunizations are dated 9/7/2022.

Plan of Correction: The parents should have it because the child went with their sibling and they both had appointments. I will ask the parents to bring the most recent shots.

Standard #: 8VAC20-800-320-B
Description: Based on observation, protective covers were not installed on all unused electrical outlets.
Evidence: There was an unused outlet without a protective cover observed above the sink in the children's bathroom.

Plan of Correction: Corrected during the inspection.

Standard #: 8VAC20-800-430-A
Description: Based on observation, children under two years of age had access to stairs.
Evidence: There is a downward staircase that the children have access to where the program takes place. There is a gate at the top of the stairs, however the gate is only secured on one side and swings open without a hook, latch or lock. There were two children observed today under the age of two.

Plan of Correction: I will latch the gate so it's blocked on both sides.
