Technical Assistance:
Technical assistance was offered related to: Handwashing, floor space used by children, background checks, emergency drills.

Comments:
A monitoring inspection was conducted on 5/8/23 from 2:05 pm to 3:15pm.There were 3 preschool children present (6 points) with 1 caregiver supervising. The children were observed playing with home living toys and other materials for free play, and eating afternoon snack. 3 children's records, 1 caregiver's record and 1 household member's record was reviewed. There were no medications on site. A walk through of the family day home was conducted to assess the physical space used by the children.

Violations:
Standard #: 8VAC20-800-830-A
Description: Based on documents reviewed, emergency evacuation procedures were not practiced monthly with all caregivers and children in care.
Evidence: A review of the family day home's drills log showed that evacuation drills were not conducted for October 2022 and April 2023.

Plan of Correction: Began a new logging system.

Standard #: 8VAC20-800-100-A
Description: Based on records review, the provider did not obtain documentation of a physical examination for a child enrolled within 30 days after the first day of attendance.
Evidence:
1. Child # 1's first day of attendance was 9/14/21.
2. Child # 2's first day of attendance was 12/1/22.
3. Child # 3's first day of attendance was11/28/22.
4. Documentation of a physical exam has not been obtained for the three children to this date.

Plan of Correction: Gave parents forms to fill out.

Standard #: 8VAC20-800-50-A
Description: Based on staff interview, the family day home did not maintain a written record of children in attendance each day.
Evidence: The provider stated that she did not maintain current attendance for children in care since 4/25/23.

Plan of Correction: Took attendance and caught up past delay.

Standard #: 8VAC20-800-90-A
Description: Based on record review, the provider did not obtain documentation of immunization for children prior to their first day of enrollment.
Evidence:
1. Child # 1's first day of enrollment was 9/14/21.
2. Child # 2's first day of enrollment was 12/1/22.
3. The provider has not obtained immunization records for both children to this date.

Plan of Correction: Gave parents forms to fill out.

Standard #: 8VAC20-800-280
Description: Based on observation, cleaning materials and chemicals were accessible to children.
Evidence:
1. The sink under the kitchen counter where a child was observed washing hands was not locked. There were several cleaners stored in that cabinet.
2. A bottle of air freshener was sitting on a kitchen table. Children have access to the kitchen.

Plan of Correction: Locked under sinks, put all cleaners away.

Standard #: 8VAC20-800-320-B
Description: Based on observation, electrical outlets were not covered by child-resistant protective covers.
Evidence: 5 electrical outlets were not covered by protective covers in the basement where children were observed playing.

Plan of Correction: Covered outlets.
