Comments:

The inspection was initiated on May 2, 2023 and concluded May 9, 2023. There were 5 children present, ranging in ages from 7 months to age 3, with 2 staff supervising. The inspector reviewed compliance in the areas of administration, personnel, physical health of caregivers, caregiver training, physical equipment/ environment, care of children, preventing spread of disease, emergencies and nutrition. A total of 5 child records,1 household member, 1 provider and 1 assistant record were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program

Description:	Based on record review and provider interviews, the provider did not ensure the results of the Central Registry Search (CRS) were received by the end of the 30th Day of employment for all providers. Evidence: The record for Provider #2 did not contain a CRS and thy began working on 2-28-23.

Description:	Based on record review and provider interviews, the provider did not ensure to obtain the parent's written acknowledgement of the receipt of the required written information for parents regarding the family day home. Evidence: The records for Child #1 (date of attendance 10-1-23), Child #2 (date of attendance 9-26-22) and Child #4 (date of attendance 9-19-22) did not include written acknowledgements from parents.

Description:	Based on observation and provider interviews, the provider did not ensure that when toddlers and infants use play pens the play pen shall not have torn mesh sides or vinyl-covered or fabric-covered rails, protruding rivets on the rails, or broken hinges. Evidence: Provider #1 confirmed the play pen was used last week for a toddler and there were 2 holes observed in the mesh sides.
