An unannounced renewal inspection was conducted on 05/10/2023 from 1:55pm to 2:45pm. There was 1 child age 15 years old (0 points) with the Provider. The programming area, attendance, outdoor playground area, menus, Provider record, 4 children?s records, emergency drills, emergency supplies, and policies were reviewed. Child was observed being dropped off by the school bus after school. There Provider had current certification in CPR and First Aid training. Areas of non-compliance are identified in the Violation Notice.
Please complete the columns for "Plan of Correction" and "Date to be Corrected" for each violation cited on the Violation Notice, and then return a signed and dated copy to the licensing office by 5pm on 5/15/23. Please email me at angela.dudek@doe.virginia.gov with any questions.

Standard #: 22.1-289.036-A Description: Based on review of the Caregiver record, the Provider did not obtain a criminal history background check report every five years from the date of the last check.
Evidence: The record for the Caregiver #1 did not contain documentation that a criminal history background check report had been completed every five years from the date of the last check. The last criminal check was completed 3/14/18.Plan of Correction: Made an appointment on May 11,2023 to get criminal background check.
