Comments:
A renewal inspection was initiated on 5/11/2023 and concluded on 5/11/2023. There was one child present with one provider. The point total was two. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of two child records and one staff record was reviewed. The inspection started at 10:15 am and concluded at 12:15 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-800-760-A-1
Description: Based on observation the provider failed to ensure that the first aid kit contained at a minimum adhesive tape and a second triangular bandage.
Evidence:
The first aid kit did not have adhesive tape or a second triangular bandage.

Plan of Correction: The provider will purchase adhesive tape and a second triangular bandage for the first aid kit.

Standard #: 8VAC20-800-770
Description: Based on observation and interviews the provider failed to ensure that a working battery-operated flashlight, a working portable battery-operated weather band radio, and extra batteries shall be kept in a designated area and be available to caregivers at all times.
Evidence:
The provider does not have a battery-operated weather band radio, and extra batteries.

Plan of Correction: The provider will get the weather band radio and make sure extra batteries are available.

Standard #: 8VAC20-800-210-A
Description: Based on record reviews the provider failed to ensure that In addition to satisfactory completion of first aid training and CPR training, caregivers shall obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities.
Evidence:
The provider has six hours of annual training.

Plan of Correction: The provider will ensure that 16 hours of annual training are received for this licensure period. She will check the online trainings that are available.

Standard #: 8VAC20-800-240-A
Description: Based on observation the provider failed to ensure that Areas and furnishings of the family day home, inside and outside, shall be maintained in a clean, safe, and operable condition. Unsafe conditions shall include, but not be limited to, the presence of poisonous plants; tripping hazards; unstable heavy equipment, furniture, or other items that a child could pull down on himself.
Evidence:
Located in the bedroom on the right on top of a chest of drawers that is approximately three feet tall is an unsecured 44 inch flat screen TV. This item could be pulled off or fall off on a child.

Plan of Correction: The provider will have a family member secure the TV and all items deemed a tipping hazard.

Standard #: 8VAC20-800-280
Description: Based on observation the provider failed to ensure that Potentially poisonous substances, materials and supplies such as, but not limited to, cleaning agents, disinfectants, deodorizers, plant care chemicals, pesticides, and petroleum distillates shall be stored away from food in areas inaccessible to children.
Evidence:
In the bathroom on top of the toilet tank was a spray can of Lysol and Levo. Both accessible to the child in care.

Plan of Correction: The provider will ensure that all hazardous substances are kept out of the reach of children or locked.
