Technical Assistance:

Discussed with the provider requirements for transporting children from the residence to include additional requirements if the provider is transporting children in a vehicle. If children are transported children should be secured in an an individual care seat or an individual and appropriate restraint in accordance with Virginia law. The provider must send documentation to the LI that the background checks have been requested no later than 10 days following this notification dated 5/15/2023. Once the background check is received by the provider, the results must be forwarded to the licensing inspector.

Comments:

A complaint investigation was conducted on 5/15/2023 in response to a complaint received by the Fairfax Licensing Office on 5/11/2023 in regards to the improper transportation of children. Interviews were conducted with the provider and records were reviewed. The information gathered during the investigation supports the allegation, and the complaint is determined to be "valid.? Description:	Based on record review, the provider did not obtain general written permission from the parent of each child for the provider to take the child off premises of the family day home. Evidence: 1. On 5/11/2023 Caregiver 2 took Child 1 off premises in her personal vehicle to run a personal errand. 2. Child 1's record did not contain written permission from the child's parent for the provider to take the child off the premises of the family day home.

Description:	Based on record review and provider statement, the substitute provider did not record and sign the time of arrivals and departures on each day that the substitute provider works. Evidence: On 5/11/2023 Caregiver 1 was acting in the capacity as the substitute provider while the provider was absent from the family day home. There was no record to document Caregiver 1's arrival and departure from the role of substitute provider for 5/11/2023.
