Comments:
An unannounced inspection was conducted on 04/27/2023 between the hours of 1:10pm and 2:30pm in response to a complaint that was received by the licensing office on 04/25/2023 relating to operational responsibilities. . Observation and an interview were conducted with the provider. Based on the preponderance of evidence, the complaint is determined to be valid. Violations:
Standard #: 8VAC20-800-30-E
Complaint related: Yes
Description: Based on observation and provider interview, the provider did not ensure that services were conducive to the welfare of one child in care.

Evidence:
1. Upon arrival at the Family Day Home on 4/27/2023, Child #1, age 2 months, was observed sleeping in a crib with an item of clothing, an adult sized shirt, completely covering the child's face, to include his nose and mouth.
2. Child #1 was observed in a bouncer during which the provider was observed to place the shirt over the child's face, then holding in place the child's pacifier which was located under the shirt, and bouncing the child in the chair, as it is designed to do, in an effort to provide comfort to the child.
3. Provider stated the shirt was provided by the parent to provide comfort to the child as the child was comforted by the parent's scent and the provider had been placing the shirt on the child's face in an effort to help him sleep.

Plan of Correction: Provider requested additional information regarding safe sleep practices.
Provider removed the shirt during the inspection and stated that she would use the shirt in a manner that prevents the shirt from covering the infant's face.

Complaint related: No
Description: Based on observation, two cribs used by infants did not have a tight fitting bottom cover.

Evidence:
1. Child #1 and Child #2 were both observed sleeping in their assigned cribs.
2. The bottom cover in the cribs occupied by Child #1 and Child #2 were not tight fitting. The bottom covers in both of the cribs were observed to be loose, allowing material to gather on the surface of the mattress.

Plan of Correction: I will buy sheets that fit tighter on the mattress.
