Technical Assistance:
Consultation was given on the difference between points and capacity and the need to stay within required terms of the license. The provider reached out to the Licensing Administrator as well for clarification.

Comments:
An unannounced monitoring visit was conducted. Eight children were present for 19 points with one caregiver which exceeds the maximum capacity and points. Children were observed in free play in the dedicated space in the basement as well as one infant sleeping upstairs in a crib. Reviewed 3 children's records, background checks and TB documentation for caregiver and household member, training, emergency preparation and physical space. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 8VAC20-800-700-A
Description: Based on observation and record review, a non-prescription topical cream did not have the written authorization from the parent.
Evidence:
Child #2 has 2 diaper creams on-site and the file did not have written authorization for the provider to administer the diaper cream.

Plan of Correction: The form was returned and put in file. A lot of things were moved during inspection. It's been done.

Standard #: 8VAC20-800-100-A
Description: Based on record review, documentation of a physical within 30 days of enrollment was not available.
Evidence:
Child #1 re-enrolled in April 2023 and the provider has not moved the physical exam documentation into the current child's record.

Plan of Correction: Not available online. Contact Inspector for more information.

Standard #: 8VAC20-800-40-A
Description: Based on observation and provider statement, the provider failed to ensure that the total number of children receiving care at any one time did not exceed the maximum licensed capacity of the home.
Evidence:
The license is currently issued for seven children. On the day of the inspection, eight children were present. After a conversation with the provider she stated that she enrolled and cares for up to 10 children since "the end of March". A review of the attendance sheet confirmed that on 4/21/23 ten children were present.

Plan of Correction: I typically only have 8 kids-not 10. If siblings and or parents are here, I write them down still. Learned that from the fire marshal. Also might have been an instance where a couple left and a couple came.

I will have one leave until numbers drip in Fall. They are looking for care.

Standard #: 8VAC20-800-570-A
Description: Based on observation and a review of children's records, the provider failed to ensure that the allowable points per caregiver did not exceed sixteen points.
Evidence:
On 5/15/2023 eight children were present with one caregiver for a total of 19 points. No additional qualified caregiver was on-site to assist with supervision.

Plan of Correction: Hiring my nephew as an assistant for the summer. Not necessary if child above leaves as then numbers drops as well. (3 points)

Standard #: 8VAC20-800-690-E
Description: Based on observation, a soiled diaper was not disposed of in a storage system that was either foot operated or used in such a way that the the caregiver's hands does not touch the exterior of the surface.
Evidence:
A soiled diaper was sitting on the kitchen floor by the trash can upon the Inspector's arrival. The Inspector observed the provider open the trash can door with her hand and drop the diaper into the can. It was not operated in a way that the caregiver's hand did not touch the exterior of the trash can during disposal. The provider stated she does not use the foot operated can downstairs to dispose of soiled diapers because it makes the room smell.

Plan of Correction: A bit misunderstood. I have one baby that is currently having "issues" and I prefer all of us not smell hers. I double bag it for germs and smell and put it upstairs out of harms way. Basement trash can is foot operated and used.
