Technical Assistance:
Technical Assistance discussed with the provider: bottle labels when more than one infant are in care; removing equipment that was discussed as not in use; shelter-in-place drills; and menus.

Comments:
An announced initial inspection was conducted on 5/9/23 from 10:20am to 12:30pm with the provider. The provider plans to operate Monday through Friday from 6:30am to 5:00pm. The provider will care for up to 12 children from six-weeks-old to 12-years-old. There were three children in care, ranging in age from five-months to three-years-old, for a total of 10 points. Children were observed playing with blocks, cars, and bubbles, singing and dancing, and an infant was fed a bottle and had their diaper changed. Four child records, the provider?s record and three household members? records were reviewed. The provider has current certification in CPR and First Aid. Medications will be administered upon parental request, appropriate authorization, and in accordance with the program's medication policy and procedures. One medication and the corresponding authorization form were reviewed and the provider has current certification in Medication Administration Training (MAT). The home is equipped with a variety of age-appropriate materials. Meals are provided by the provider. Policies and procedures were reviewed. The first aid kit, flashlight, smoke detectors, and the carbon monoxide detector were observed. Other nonmedical emergency supplies were discussed. Emergency numbers were posted. A conditional (6-month) license will be recommended. An unannounced follow-up inspection will be conducted within 60 days to determine compliance with standards not observed at the initial inspection. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Description: Based on observation, the provider did not ensure that the first aid kit contained all of the required items. Evidence: On 5/9/23 the first aid kit did not contain scissors, tweezers, adhesive medical tape, gloves, ice packs, or triangular bandages (2).

Description: Based on observation and interview, the provider did not obtain a working portable battery-operated weather band radio, and extra batteries. Evidence: The provider reported that they did not obtain a battery-operated weather band radio.

Standard #: 8VAC20-800-800-A
Description: Based on interview, the provider did not ensure that the family day home had a written emergency preparedness and response plan that 1. Included emergency evacuation, emergency relocation, and shelter-in-place procedures; 2. Addressed the most likely to occur scenarios, including but not limited to fire, severe storms, flooding, tornadoes, and loss of utilities; and 3. Included provisions for a responsible person who is 18 years of age or older and is able to arrive at the family day home within 10 minutes for emergency backup care until the children can be picked up by their parents. Evidence: The provider reported that there was not an emergency response plan prepared.
