Technical Assistance:
The next anticipated inspection will be the providers renewal inspection. Provider was reminded to have all renewal application documents submitted through the provider portal 45 days prior to expiration for processing. Links to the portal can be found on the VDOE website.

Comments:
An unannounced monitoring inspection was conducted today between 9:40am and 11:15am. There were 6 children present with the Provider and 1 assistant for a total of 16 points within the supervision guidelines. The children were observed playing freely, singing songs, eating a morning snack (blueberries and crackers with milk or water) and playing outside. A complete inspection of the physical plant, children and staff records, menu, attendance and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Violations:
Standard #: 8VAC20-800-750-A
Description: Based on record review and interview, the provider did not obtained authorizations to apply topical skin products prior to using on the children.
Evidence: Child #4 and Child #5 were observed having sunscreen applied. The provider did not have authorizations to apply the sunscreen.

Plan of Correction: I gave the form to the parents but they didn't return it. I will ask them to return the form.

Standard #: 8VAC20-800-750-D
Description: Based on observation, product were used be used beyond the expiration date.
Evidence:
1. Child #4 was observed having sunscreen applied before going outside to play. The child's sunscreen expired 9/2021.
2. Child #5 was observed having sunscreen applied before going outside to play. The child's sunscreen expired 8/2022.

Plan of Correction: I'll have to start checking the dates.

Standard #: 8VAC20-800-800-B
Description: Based on record review, the provider has not reviewed the emergency plan at least annually.
Evidence: The most recent review of the emergency preparedness plan was dated 8/5/2020.

Plan of Correction: I will add it to a list of things to do.

Standard #: 8VAC20-800-830-C
Description: Based on record review and interview, documentation of emergency evacuation and shelter-in-place drills was not maintained.
Evidence: The most recent documented emergency drill was dated 11/18/2022. The provider stated that they must have forgotten to write them down.

Plan of Correction: I'll write it down as I perform them.

Standard #: 8VAC20-800-920
Description: Based on record review, menu requirements were not met.
Evidence: The weekly menu was not dated.

Plan of Correction: I will start dating it as I make it.

Standard #: 8VAC20-800-960-E
Description: Based on observation, bottles were not labeled with the child's full name and the date.
Evidence: Two bottles were observed with Child #1's name. The bottles were not dated.

Plan of Correction: I will let the parent know to start doing it.

Standard #: 8VAC20-800-60-B
Description: Based on record review, children's records did not contain all required information.
Evidence:
1. Child #2's record does not contain the families medical insurance policy number.
2. Child #6's record does not contain the child's first date of attendance and the families medical insurance policy number.
3. Child #6's record does not contain the families medical insurance policy or policy number.

Plan of Correction: I will text the parents and ask them to fill the information in.

Standard #: 8VAC20-800-90-C
Description: Based on record review, the families has not obtain documentation of additional immunizations for each child under the age of two years.
Evidence: Child #1 (11 months) most recent immunizations are dated from when the child was 2 months old.

Plan of Correction: I'll talk to them today.

Standard #: 8VAC20-800-180-A
Description: Based on record review, each caregiver and/or adult household member has not obtained a repeat tuberculosis (TB) screening every two years.
Evidence: Household member #1's TB screening expired on 12/5/2022 and has not been renewed.

Plan of Correction: They have made an appointment to get it done.
