Technical Assistance:
Today we discussed documentation of annual training. Caregivers are required to obtain a minimum of 16 hours each year.
The license is completing and tracking training per calendar year for all caregivers and will organize and document the training for each calendar year grouping the trainings accordingly for review.

Comments:
A monitoring inspection was initiated and concluded on 5/12/2023. There were 6 children present with the provider. The point total was 16. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 5 caregiver / household member records were reviewed. The inspection started at 1:20 pm and concluded at 2:40 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law as well as repeat violations.
Violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-800-80-B
Description: Based on record review and interview, the licensee did not always verify the identity and age of each enrolled child by viewing acceptable documentation.
Evidence:
Four Children's Records were reviewed,
The record for child #3 whose date of enrollment was documented as 9/7/2021did not contain documentation that the provider verified the identity and age of child #3,
The provider stated that she had requested the documentation but did not have documentation that she had viewed it.

Plan of Correction: The licensee will request that the parent provide documentation of proof of identity documentation for child #3 within 7 days.

Standard #: 8VAC20-800-80-D
Description: Based on a review of records and interview, the provider failed to notify local law-enforcement that a parent did not provide the required proof of identity for an enrolling child within seven business days of the child's first day of attendance at the family day home.
Evidence:
Four Children's Records were reviewed,
The record for child #3 whose date of enrollment was documented as 9/7/2021did not contain documentation that the provider verified the identity and age of child #3,
The provider confirmed that she had requested the documentation but had not viewed it and had not notified law enforcement that she was unable to view proof of identity of child #3 within 7 days of enrollment in her family day home.

Plan of Correction: The provider will notify local law-enforcement that the parent failed to provide the required proof of identity for an enrolling child within seven business days of the child's first day of attendance at the family day home.

Standard #: 8VAC20-800-90-A
Description: Based on a review of records and interview, the licensee was unable to demonstrate that she always obtained documentation that each enrolling child has been adequately immunized.
Evidence:
Four children's records were reviewed.
The record for child #4, whose date of enrollment was documented as 10/24/2022, did not contain documentation of immunizations or documentation demonstrating that the child had been adequately immunized.
The licensee stated that she did not realize this documentation was not in child #4's record.

Plan of Correction: The licensee will request that the parent of child #4 provide proof of adequate immunization within 7 days in order to continue attendance at the family .day home

Standard #: 8VAC20-800-180-A
Description: Based on a review of records and interview, the license did not obtain from each caregiver and adult household member a current report of Tuberculosis screening results within 2 years of their last Tuberculosis Screening.
Evidence:
Four Provider / Assisitant / Household records were reviewed.
The record for Caregiver #1 contained the most recent Tuberculosis screening which was dated 1/25/2021.
The record for Caregiver #2 contained the most recent Tuberculosis screening which was dated 1/16/2021.
The record for Caregiver #3 contained the most recent Tuberculosis screening which was dated 1/25/2021.
The licensee confirmed that she did not have more recent Tuberculosis screening results for caregiver #1, #2 and #3.

Plan of Correction: The licensee will require that each caregiver and adult household member have a current Report of Tuberculosis Screening conducted and submit documentation within 2 weeks.

Standard #: 8VAC20-800-350-E
Description: **REPEAT VIOLATION**
Based on thermometer readings taken at the time of inspection, the licensee did not ensure that hot water taps available to children are maintained within a range of 105 to 120 degrees Fahrenheit. Evidence: The water temperature at the handwashing sink in the restroom that children utilize while in care measured 124.2 degrees Fahrenheit with a digital thermometer at the time of the inspection on 5/12/2023.

Plan of Correction: The provider will either turn off the hot water tap in the bathroom or adjust the temperature on the hot water heater, today, to lower the temperature of the hot water to 120 degrees Fahrenheit or cooler in the restroom where children wash their hands. The Hot water tap will be monitored regularly to ensure the temperature remains lower than 120 degrees Fahrenheit.
