Comments:
A monitoring inspection was conducted on 05/17/2023 between the hours of approximately 11:40 am and 12:45 am. There were 12 children present with 3 caregivers supervising.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records, 3 staff records and 1 household member record were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations:
Standard #: 22.1-289.011-F
Description: Based on review of required posted documents, the provider did not ensure that documents required by the Superintendent were posted.

Evidence:
1) On the date of inspection, the most recent license for the program was not posted. The license posted had expired in January 2023.

Plan of Correction: Corrected. The provider printed and posted the license for the current licensing period.

Standard #: 8VAC20-800-920
Description: Based on interview with the provider and documentation review, the menu for meals and snacks provided by the family day home did not meet all requirements.

Evidence:
1) On the date of inspection, the menu available for review was not dated for the current one-week period. The date corresponded to April 2023.

Plan of Correction: Done. Did update the date.

Standard #: 8VAC20-800-50-A
Description: Based on documentation review, the provider did not ensure to keep a written record of children in attendance each day.
Evidence:
1) On the date of inspection, the record of attendance was not current. The weekly attendance record showed attendance for 05/15 and 05/16. The attendance for 05/16 and 5/17 was not recorded.

Plan of Correction: Corrected. The provider updated the attendance record during the inspection.

Standard #: 8VAC20-800-60-A
Description: Based on record review, children's records did not contain all required information.
Evidence:
1) The record for Child #2 did not contain the address of one custodial parent's place of employment; and the address of a second designated person to contact in case of an emergency if the parent cannot be reached.

Plan of Correction: Will follow up with the parent and update.

Standard #: 8VAC20-800-320-B
Description: Based on observation, the provider did not ensure that all unused electrical outlets have child-resistant protective covers.
Evidence:
1) One outlet in the bathroom of the care area did not have a protective cover.

Plan of Correction: Will call electrical and fix it.

Standard #: 8VAC20-800-540-A
Description: Based on observation, safety straps were not fastened when using infant seats.
Evidence:
1) On the date of inspection, an infant was positioned on a bouncy chair and the safety straps were not fastened.

Plan of Correction: Corrected. The provider fastened the safety belt during the inspection. The provider shall ensure, moving forward, that safety belts are fastened at all times while an infant is using the bouncy chair.

Standard #: 8VAC20-800-540-B
Description: Based on observation, infant walkers were used.
Evidence:
1) On the date of inspection, while the children were playing outside, the inspector observed two children in infant walkers. While inside the fdh, the inspector observed one child in an infant walker.

Plan of Correction: Done.
We removed the wheels of the walker which is outside. The walker inside doesn't have wheels, we are just using it as a standing toy. (LI has requested to not use any of the walkers until we get clarification from management.)
