Technical Assistance:

The UMI conducted on 11/03/2022 was signed today. This past inspection was never received within the Office of Health & Safety (Fairfax). The current FDH Renewal Application was submitted to the Licensing Inspector. Information was given in reference to obtaining information through our website at: www.childcare.virginia.gov (for training and model forms).

Comments:

This renewal inspection was initiated by licensing staff and concluded on 05/18/2023, between the hours of 10:30 am to 1 pm. There was 1 preschool-age child present (2 points) with 2 caregivers) supervising. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; special care and emergencies; and nutrition. A total of 7 children records, 3 household records, and 2 staff records were reviewed. Information gathered during the inspection determined that there were (5) non-compliances with the applicable standards. These violations were documented on the Violation Notice issued to the home. Standard #:	8VAC20-800-100-A
Description:	Based on review, 6 of 7 children files indicated that the Provider did not obtain documentation of a physical examination (by or under the direction of a physician) within 30 days after the first day of attendance. Evidence - 1. The following children were without documentation of a physical on file: Child #1 (start date - 06/08/2012), Child #2 (start date - 08/22/2014), Child #3 (start date - 12/27/2018), Child #5 (start date - 09/11/2020), Child #6 (start date - 08/09/2017), and Child #7 (start date - 01/19/2023). 2. These dates were review by the Licensing Inspector with the Provider on the date of inspection.
Plan of Correction:	The missing physicals will be obtained.
Standard #:	8VAC20-800-60-B
Description:	Based on review, 4 of 7 children files indicated that a written authorization for emergency medical care (should an emergency occur and the parent cannot be located immediately). Evidence - 1. On the date of inspection (05/17/2023), the following children did not have a complete authorization for emergency medical care on file at the FDH: Child #1, Child #2, and Child #3. Page 2 of the Child's Record was not available. 2. The authorization for emergency medical care for Child #6 was missing the parent signature. 3. There was not a parent's written objection to the provision of medical treatment on religious or other grounds on file for any enrolled child.
Plan of Correction:	The authorization for emergency medical care will be obtained for all children.
Standard #:	8VAC20-800-70-A
Description:	Based on review, 3 of 7 children files indicated that not all documentation was on file within each child's record. Evidence - 1. On the date of inspection (05/18/2023), page 2 of the Parent Information Form was not available for: Child #1, Child #2, and Child #3. 2. On the date of inspection, page 2 of the Administration of Medication Policy Form was not available for: Child #1, Child #2, and Child #3.
Plan of Correction:	The missing information/documentation will be completed and placed on file.
Standard #:	8VAC20-800-210-A
Description:	Based on review, 0 of 2 staff files indicated that each caregiver obtained a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities. Evidence - 1. On the date of inspection (05/18/2023, a total of 8 hours training for Staff #1, and a total of 8 hours of training for Staff #2 was obtained for the past annual training period. 2. These hours were calculated after review of certificates kept on file for each staff person of the past annual training period.
Plan of Correction:	A total of 16 hours of annual training for each staff person will be obtained.
Standard #:	8VAC20-800-460-C
Description:	Based on observation and review, the shot record for the pet observed on site was not available. Evidence - 1. On the date of inspection (05/18/2023), a dog (Mallo) was observed on site. The Provider stated that it belonged to a household member. 2. The vaccination record for this animal was not available for review on the date of the inspection. 3. The dog was separated; kept behind a barrier and upstairs from the daycare program located downstairs in the Family Day Home.
