Comments:
An unannounced renewal inspection was conducted today, May 2, 2023. The inspector was on site from approximately 12:05 pm until 1:40 pm. There were six children present with one caregiver supervising and a volunteer for a value of 12 points. Children were observed playing with riding toys, the kitchen set and cars. Later children had lunch and prepared for nap. The provider sat and conversed with children during lunch. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of six children?s records and one caregiver record and two household member records were reviewed. The inspectors interacted with the caregiver and children in care.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.036-A
Description: Based on review of two household member's records and interview, the provider did not obtain a current central registry check every five years from the initial central registry check for one household member.

Evidence:

1. The most recent central registry check for household member #2 was dated 02/20/2018.
2. The provider stated she had not requested an updated central registry for household member #2.

Plan of Correction: Per the provider: I will have my daughter help we send off the request to update the central registry check.

Standard #: 8VAC20-800-730-D
Description: Based on observation, the provider did not ensure medication was kept in a locked place using a safe locking method that prevents access by children.

Evidence:

A box containing children?s Tylenol was observed sitting on the table in the den. Child #6, age 11 years, in care as observed in the den with access to the medication.

Plan of Correction: Per the provider: I will lock up the medication immediately.

Standard #: 8VAC20-800-60-B
Description: Based on review of six children?s records, the provider did not obtain all required documentation for each child?s record.

Evidence:

1. The record of child #1, enrolled 03/20/2023, did not contain parent?s signed acknowledgement of the receipt of whether there is liability insurance.
2. The record of child #2, enrolled 04/13/2020, did not contain the name, address and phone number for two designated persons to be contacted in an emergency if the parent cannot be reached.
3. The record of child #4, enrolled 11/15/2021, did not contain the parent work pho e number and address.
4. The record of Child #6, enrolled 04/24/2023, did not contain the name, address and telephone number of the child?s physician.

Plan of Correction: Per the provider: I will have the parents complete all the missing information for each child's record.

Standard #: 8VAC20-800-90-A
Description: Based on review of six children?s records, the provided did not obtain documentation that a child was adequately immunized, before the child attended the family day home.

Evidence:

The record of child #6, enrolled 04/24/2023, did not contain documentation of immunizations exceeding the first day of attendance.

Plan of Correction: Per the provider: I will have the parent bring me a copy of the immunization record for the child's file.

Standard #: 8VAC20-800-260-A
Description: Based on observation, the provider did not ensure the water of the aboveground swimming pool was not accessible to children in care.

Evidence:

The privacy fencing surrounding the raised deck of the aboveground pool had a rear gate that was missing a piece and could not be secured shut. The open area was approximately four feet off the ground and had no steps leading the pool however there was a pool pump that could be used to climb onto the deck and access the water. The entrance gate to the pool was locked.

Plan of Correction: Per the provider: I will have my son repair the fence and secure it so that it's locked. Children do not play near the pool.
