Technical Assistance:
Discussed with the provider that the policy for administering medication needs to be updated. Provider is not MAT trained.

Discussed the provider needs to obtain Name and location of previous day care programs and schools the child has attended within 7 business days of the child's first day of attendance at the family day home.

Discussed the provider will need a child's results of their health exam within 30 days of enrollment.

Discussed the playground equipment and spacing.

Comments:
An announced initial inspection was conducted today from 9:35am-11:04am with the provider on site. There were 3 children with 2 caregivers. A sample of 3 children's files and 4 caregivers/household member's files were reviewed. The children were engaged in the following: toy play, coloring, playing outside and drinking a bottle. Interactions between the children and caregivers were positive. The site was clean, organized and contained an abundant supply of developmentally appropriate materials. The physical space, attendance records, first aid kit and the Emergency Preparedness Plan were reviewed. There were no medications on site. Description: Based on interview, the provider did not have a carbon monoxide detector.
Evidence:
1. The provider stated they did not have a carbon monoxide detector.

Plan of Correction: Buy a carbon monoxide detector.

Standard #: 8VAC20-800-770
Description: Based on observation, the provider did not have a portable battery operated weather band radio.
Evidence:
1. The provider had a battery operated radio, but it was not a weather band radio.

Plan of Correction: Buy a battery operated radio.

Standard #: 8VAC20-800-790
Description: Based on observation, the provider did not post all required telephone numbers.
Evidence:
1. The provider did not have the responsible person (who is 18 years of age or older and is able to arrive at the family day home within 10 minutes for emergency backup care until the children can be picked up by their parents.) for emergency backup care posted with their name and number.

Plan of Correction: The required telephone number with a person's name has been posted in the premises.

Description: Based on observation, the provider allowed a child to drink a bottle while lying down.
Evidence:
1. The provider laid a child down in the crib and gave the child a bottle.

Plan of Correction: Ensure never to give the child a bottle during laying down. I put him in a high chair when give him bottle.

Standard #: 8VAC20-800-60-B
Description: Based on review, three children's records did not contain all required information.
Evidence:
1. Child #1's file (unknown start date) was missing both parents work address and work phone number, the doctor office address and phone number, one emergency contact name, address and phone number and another emergency contact address only, medical insurance name and policy number, 1st day of attendance.
2. Child #2's file (Start date 8/15/2022) was missing the doctor office address, and one emergency contact name, address and phone number.
3. Child #3's file (Start date 3/17/2022) was missing one parent's work address, two emergency contact addresses, medical insurance and policy number , review of emergency contact information.

Plan of Correction: Need child record form has been given to the parents (Child 1, 2, 3) and have already ask them to submit the forms with missing information by 5/26.

Standard #: 8VAC20-800-90-A
Description: Based on review, the provider did not obtain before a child attended the family day home, documentation that the child has been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health.
Evidence:
1. The provider had an immunization record for child #2, but it was not signed by a physician to document the child has been adequately immunized.

Plan of Correction: Return the immunization form to child 2 parents and have asked them to get the formed signed by the physician on 5/24.

Standard #: 8VAC20-800-240-A
Description: Based on observation, furnishings of the family day home inside were not maintained in a safe and operable condition.
Evidence:
1. One of the provider's cribs had paint peeling on both rails.

Plan of Correction: Remove the crib from the premises. It was an extra crib.

Standard #: 8VAC20-800-500-C
Description: Based on observation, the provider had stationary outdoor playground equipment that was not placed at least six feet from the perimeter of other play structures or obstacles.
Evidence:
1. The provider's playground equipment was approximately 4 feet from the fence and 2 feet from a slide.

Plan of Correction: Remove the outdoor playground equipment from the current location and reinstall it at least six feet from the perimeter of other play structures.
