Comments:

An announced initial inspection was completed on 5/22/2023 between the hours of 11:30am to 1:30 pm. There were two children present with the provider. The provider was warm and encouraging with the children as they engaged in free play. One crib was inspected, along with an adequate amount of cots for napping. A variety of age appropriate materials were also observed in the family day home. The Inspector reviewed 3 children's records, 3 caregiver/household member records, and the Emergency Preparedness Plan submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Description:	Based on record review, 3 of 3 children's records reviewed did not contain all required information. Evidence: 1. The record for Child #1 and Child #3 did not contain the name and policy number of the child's medical insurance. 2. The record for Child #1 and Child #3 did not contain 1 of 2 required two designated persons to contact in case of an emergency if the parent cannot be reached. 3. The record for Child #1 and Child #3 did not contain the address and telephone number of each custodial parent's place of employment.
Plan of Correction:	I will have the parents fill out the information.
Standard #:	8VAC20-800-70-A
Description:	Based on record review and provider interview, the parents were not provided all required information in writing before the child's first day of attendance. Evidence: 1. The records for Child #1, #2, and #3 did not contain documentation that the parents received written confirmation of whether or not there is liability insurance on the family day home operation.
