2. The record for Child #1 and Child #3 did not contain 1 of 2 required two designated persons to contact in case of an emergency if the parent cannot be reached.
3. The record for Child #1 and Child #3 did not contain the address and telephone number of each custodial parent's place of employment.Plan of Correction: I will have the parents fill out the information.
Standard #: 8VAC20-800-70-A Description: Based on record review and provider interview, the parents were not provided all required information in writing before the child's first day of attendance.
Evidence:
1. The records for Child #1, #2, and #3 did not contain documentation that the parents received written confirmation of whether or not there is liability insurance on the family day home operation.Plan of Correction: I have the form and will have the parents sign.
