Comments:
An unannounced monitoring/renewal inspection was conducted today from 10:30am-11:10am. There were 9 children directly supervised by 3 staff. The home, 3 staff records, 5 children?s records, 1 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely and eating. There was an assortment of age appropriate materials for children. The home was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Violations:
Standard #: 8VAC20-800-980-A
Description: Based on record review, not all children's record contained general written permission shall be obtained from the parent of each child for the provider to take the child off the premises of the family day home.
Evidence:
Child 4's record did not contain general written permission for transportation. The provider stated that the children take regular walking trips to a nearby park to play.

Plan of Correction: Child 4's parent has taken the paperwork home and will bring it back signed after the holiday weekend. Going forward a checklist for all necessary forms will be reviewed 3 times before establishing the file to be complete.

Standard #: 8VAC20-800-100-A
Description: Based on record review, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence:
Child 4's record did not contain documentation of a physical examination.

Plan of Correction: The parent has been informed to bring their child's physical examination and will hand that to me on Tuesday.

Standard #: 8VAC20-800-70-A
Description: Based on record review, parents were not provided of all required information in writing.
Evidence:
Child 4's record did not contain documentation that the parents have been informed, in writing of: the family day home's discipline policy, if there are pets present in the home, the termination policy, the medication policy, the emergency and the response plan.

Plan of Correction: The parent have been given the missing documents to review and it has been placed in Child 4's file. I double check the file and paperwork before giving it to parents to fill. From now on I will do an additional check and review my checklist when the parent returns the paperwork.
