Comments:
An unannounced monitoring inspection was conducted on 5/22/2023, from 12:15PM to 1:00PM. 10 (28pts) were directly supervised by 3 caregivers. Ratios were in compliance. The physical space, required posted documents, evacuation drills, 2 children?s records, caregiver records, the menu and attendance were reviewed. No medications were on site. Children were observed finishing lunch and preparing for rest time. Areas on non-compliance are identified on the Violation Notice. Standard #: 22.1-289.036-B-2
Description: Based on record review, criminal history records were not repeated prior to expiration. Evidence: Criminal History Searches (Fingerprints) expired: Adult 1 (expired 3/01/2023)

Plan of Correction: Provider will get this completed right away.

Standard #: 8VAC20-770-40-D-1-a
Description: Based on record review and Provider interview, the Provider did not maintain documentation of the results of the Virginia Child Abuse and Neglect Central Registry Searches. Evidence: The documentation for Caregiver 1 expired 3/22/2023 and Caregiver 2 & Caregiver 3 did not have the documentation in the file.

Plan of Correction: Provider will get these completed right away.
