Comments:
A monitoring inspection was initiated on 5/8/2023 and concluded on 5/25/2023. There were five children present on 5/8/23 totaling 16 points with one provider and seven children present on 5/25/23 totaling 21 points with two caregivers present. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and one staff record was reviewed. The inspection on 5/25/23 started at 9:50 am and concluded at 11:30 am.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-800-120-A
Description: Based on observation the provider failed to ensure that a record shall be maintained for each caregiver.
Evidence:
During the inspection a family member was providing care for the children. There was no record for this family member.

Standard #: 8VAC20-800-50-E
Description: Based on record reviews the provider failed to ensure that records and reports on children, caregivers, and household members required by this chapter shall be maintained and made accessible to the department's representative for two years from the date of termination of services for a child, date of separation from employment for caregivers, or date of termination of residence for a household member, or unless specified otherwise.
Evidence:
During the inspection on 5/8/2023 staff #1 was not home. Staff #2, substitute, was not able to access the children and caregivers records.

Description: Based on record review the provider failed to ensure that a substitute provider shall record and sign the time of arrivals and departures on each day that the substitute provider works.
Evidence:
The provider did not record the arrival and departure of staff #2 when she was a substitute provider on 5/8/23.

Description: Based on record review and interviews the provider failed to ensure that each caregiver at the time of hire and each adult household member prior to coming into contact with children a current Report of Tuberculosis Screening form published by the Virginia Department of Health or a form consistent with it documenting the absence of tuberculosis in a communicable form.

Assistant #1 does not have a tuberculosis screening on file. Assistant #1 was caring for children 5/8/23.

Description: Based on observation the provider failed to ensure that caregivers shall wash their hands with liquid soap and warm running water.
Evidence:
During the inspection on 5/8/23 assistant #1 changed two diapers on the couch. The assistant did not wash her hands following each diaper change.

Description: Based on observation the provider failed to ensure that Caregivers shall ensure that children's hands are washed with liquid soap and warm running water after toileting or diapering and after handling or caring for animals.
Evidence:
1. The assistant changed two diapers and did not wash the children's hands.
2. The provider has kittens and the children will pick them up or pet them. The children did not wash their hands after playing with the kittens.
