Comments:
An unannounced renewal inspection was conducted on 5/30/2023. The inspector was on site at the family day home from approximately 3:25pm to 5:20pm. There were 5 children in attendance and 2 caregivers present. Interviews were held with the caregiver throughout the inspection, and the inspector interacted with the children when appropriate. The yard and all areas of the home were inspected today. Two caregiver/household member records, one household member record and five children?s records were reviewed during this inspection. Information gathered during the inspection determined areas of non-compliance with applicable regulations or
law, and violations were documented on the violation notice issued to the provider. See the violation notice for more details.

Standard #: 8VAC20-800-980-A
Description: Based on record review, the provider the not
ensure five of five children?s records
contained documentation of general written
permission from parents for the provider to
take the child off the premises of the family day
home.
Evidence:
1. The record of child #1 (enrollment date:
1/30/23) did not have documentation of written
permission to be taken of the premises of the
family day home from the child?s parent. The
provider stated that the children are taken to
the for a walk around the neighborhood regularly.
2. The record of child #2 (enrollment date:
1/30/23) did not have documentation of written
permission to be taken of the premises of the
family day home from the child?s parent. The
provider stated that the children are taken to
the for a walk around the neighborhood regularly.
3. The record of child #3 (enrollment date:
2/08/23) did not have documentation of written
permission to be taken of the premises of the
family day home from the child?s parent. The
provider stated that the children are taken to
the for a walk around the neighborhood regularly.
4. The record of child #4 (enrollment date:
3/24/23) did not have documentation of written
permission to be taken of the premises of the
family day home from the child?s parent. The
provider stated that the children are taken to
the for a walk around the neighborhood regularly.

3/7/23) did not have documentation of written
permission to be taken of the premises of the
family day home from the child?s parent. The
provider stated that the children are taken to

Plan of Correction: I will have parents sign permission forms.

Standard #: 8VAC20-800-210-A
Description: Based on record review and an interview, the provider did not ensure to obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities.
Evidence:
The provider had 1.5 of the 16 required annual clock hours of training in areas relevant to their job responsibilities. During the inspection, the provider confirmed that she did not complete all of the required annual clock hours of training.

Plan of Correction: I will complete training hours ASAP.
