Comments:
A renewal inspection was initiated on 6/1/2023 and concluded on 6/1/2023. There were a total of 7 children (12 points) observed with 2 caregivers present. The children were engaged in the following: toy play, bathroom, snack. The home, 3 child records, 2 caregiver/household member records, evacuation drills, emergency supplies and playground were inspected. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations:
Standard #: 8VAC20-800-240-A
Description: Based on observation and caregiver interview, areas and furnishings of the family day home, inside and outside are not maintained in a safe and operable condition.
Evidence: The rope on the car swing on the outside playground is frayed, causing a safety hazard.

Plan of Correction: The swing will be removed until a new rope can be installed.

Standard #: 8VAC20-800-500-C
Description: Based on observation and caregiver interview, stationary outdoor play equipment is placed over a hard surface and is not placed at least 6 feet from the perimeter of other play structures or obstacles.
Evidence: The single swing is placed over the bare ground which is a hard surface and it is not 6 feet away from other play structures.
The large climber with the slide does not have a clearance of 6 feet from other play structures and obstacles.

Plan of Correction: The single swing over the bare ground will be removed from the play structure.
The equipment will be removed from around the large climber with slide to provide 6 feet clearance from any other structure.
