22.1 BACKGROUND CHECKS; CARBON MONOXIDE

Comments:
An unannounced monitoring inspection was conducted from 11:45 am - 12:45 pm. There were nine children in care being supervised by three staff for a total of 24 points. The children were observed preparing for rest time. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of four child records and three staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.035-B-2
Description: Based on review of two staff records it was determined that not all records contained documentation of a determination letter based on a fingerprint criminal history check on file before the first day of employment. Evidence: The record for Staff B (start date: 05/08/23) did not have documentation of a determination letter based on a fingerprint criminal history check on file.

Plan of Correction: Staff B will obtain a determination letter based on fingerprinting.

Standard #: 8VAC20-800-120-B
Description: Based on review of two staff records it was determined that not all records were complete. Evidence: The record for staff A did not contain the following information: name, address, verification of age, job title, date of employment, name, address, phone number of an emergency contact.

Plan of Correction: The missing information will be added to staff A's record.

Standard #: 8VAC20-800-200-A
Description: Based on review of two staff records it was determined that not all records contained documentation of orientation training within the first week of employment. Evidence: The record for Staff A (start date: 02/22/22) did not have documentation of orientation training on file.

Plan of Correction: Staff A will receive orientation training.

Standard #: 8VAC20-800-540-A
Description: Based on observation it was determined that that safety straps in high chairs were not always used when occupied by children. Evidence: Child A (age 3) was observed climbing out of a high chair. The safety straps were not fastened around the child.

Plan of Correction: Safety straps will be used whenever children occupy a seat where safety straps are intended for use.
