Comments:
An unannounced monitoring inspection was conducted on 6/1/23. The provider has one child in care totaling 4 points and other 6 children attends after school. The provider has lots of toys for the children to play with. Children bring food from home. Provider is playing with the child in care. A violation notice was given to the provider with this summary. Description: Based on the review of three children's records, the provider failed to get some of the information to make the file complete.
Evidence: Three children's records reviewed.1 child is missing one emergency contact and 2 are missing 2 emergency contacts.

Plan of Correction: I will ask the parents for the emergency contacts.
