Comments:
An unannounced monitoring inspection was conducted on 5/30/2023. There were 12 children (28 points) directly supervised by 3 caregivers. The home, 1 staff record, 2 children's records, 1 household member's records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing inside the play room. There was an abundant supply of books, toys, and materials for the children. The home was clean and organized. Areas of non-compliance are identified in this report. Standard #: 8VAC20-800-760-A-1
Description: Based on observation, the provider did not ensure that the first aid kit contained the minimum required items.
Evidence: observed that the first aid kit did not have adhesive tape or 2 triangular bandages.

Plan of Correction: I am going to replace adhesive tape or triangular bandages.

Standard #: 8VAC20-800-920
Description: Based on review, the provider did not have documentation of the menu's substituted meals.
Evidence: observed that the provider was giving the children spaghetti for lunch, but did not have spaghetti listed on the menu.

Plan of Correction: I am going to change my menu and follow up each day using from the menu.

Standard #: 8VAC20-800-120-B
Description: Based on review, the provider did not ensure that staff records were maintained with the required minimum information.
Evidence: reviewed 1 staff record and found that there was no documentation of emergency contact information.

Plan of Correction: I am going to have my staff complete the emergency contact info.

Description: Based on the provider interview, the provider did not ensure that an attendance record was maintained each day.
Evidence: identified through the provider that she had not maintained a written attendance record for the current day.

Plan of Correction: I am going to write attendance daily in the paper.

Description: Based on review, the provider did not ensure that children's records were maintained with the required information.
Evidence: reviewed 2 children's records and found that Child 1 did not have documentation of medical insurance policy numbers.

Plan of Correction: I am going to put documentation of medication the 2 child file.

Standard #: 8VAC20-800-280
Description: Based on observation, the provider did not ensure that potentially poisonous substances was kept inaccessible to children at all times.
Evidence: observed that there was 1 large container of Lysol disinfectant on a counter accessible to children and 2 Lysol spray bottles on two high chair trays, accessible to children.

Plan of Correction: I am going to make sure the Lysol disinfectant on safety and lock - so the children can not reach out.
