Technical Assistance:
Discussion of children in regards to the daily schedule and when children are placed down for nap time versus the younger children.

Discussion of new staff person and status of household members.

Comments:
This monitoring inspection was initiated by licensing staff and concluded on 05/30/2023, between the hours of 12:30 pm to 2 pm. There were 7 children present (29 points), ranging from infant to preschool-age, with 3 caregivers supervising. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; special care and emergencies; and nutrition. A total of 8 children records, 1 household record, and 4 staff records were reviewed.

Information gathered during the inspection determined that there were (2) non-compliances with the applicable standards. These violations were documented on the Violation Notice issued to the home.

Violations:
Standard #: 8VAC20-800-960-E
Description: Based on observation, not all of the required information was labeled on a bottle that was refrigerated on-site within the FDH.

Evidence -
1. On the date of inspection (05/30/23), one bottle being stored within the FDH refrigerator was labeled with the name of Child #8, but was missing today's date.
2. This was confirmed by the Provider. All other bottles were observed with both the name and date labeled on them.

Plan of Correction: Will correct.

Standard #: 8VAC20-800-90-C
Description: Based on review, 3 of 8 children files indicated that the Family Day Home (FDH) did not obtain documentation of additional immunizations for children (who were not exempt from the immunization requirements according to subsection B) once every six months for children under the age of two years.

Evidence -
1. On the date of inspection the following immunization pages were observed - For Child #1 (dated - 10/27/22), for Child #2 (dated - 10/25/22), and for Child #6 (dated - 11/09/22).
2. Child #1, #2, and #6 were all under the age of two years on the date of inspection.

Plan of Correction: Updated immunization pages will be obtained and placed on file.
